Facility Usage at Calvary Bible Church...

You may request to use our facility by filling out our Facility Request Form and submitting it to the church office. You may drop it off, email it or fax it to us.  Please expect the approval process to take at least a week.

Also, for all non-Calvary Bible Church ministry use, there are additional forms you will need to fill out once your initial request has been processed.
