You may
complete this form by using Microsoft Word or by printing it
and filling it in by hand. Please return it to the church
office either by dropping it off, email or faxing it.
The approval
process usually takes about one week from the time the form is
turned in. You will be contacted at that time to let you know
if your request has been approved.

POLICY FOR USE OF FACILITY
When
considering using a part of the facility, we must review
Article II of the Constitution of Calvary Bible Church - our
purpose. Therefore,
the use of this facility is to begin with prayer acknowledging
God as the provider of all, particularly salvation through His
Son, Jesus Christ.
The
facility may be defined as any & all of the following:
Gymnasium; Fellowship Hall; Kitchen; Kitchenette; Athletic
Field(s); Music Lab; Chapel; Conference Room; Computer Lab;
available classrooms for meetings; portable items (i.e.
tables, chairs, equipment, etc.).

The attached facility request form must be completed for each
reservation and submitted to the Church Office.
2. Any reservation conflicts will be resolved by the Ministries
Administrator.
3. The church office will contact you once the request has been
approved/denied. Until
then, the request is not approved.
4. If the original date changes to another date (or if it is canceled) once
the request form has been approved and distributed to staff
receiving copies of the form, a colored copy of a new facility
request form with the new date will be distributed to seek
approval/denial of a new date.
5. Church/school functions have absolute priority when considering
availability. Calendar reservations may be made 12 mo. to 1
week in advance of
the activity with Church office approval. It is to your
advantage to get in the attached form ASAP to be assured of
availability.

For other than church/school functions, calendar reservations may be
made 4 weeks in
advance of the activity with Church office approval.
7. Fees will be established for use of the facility.
8. The use of alcohol, tobacco, illegal substance or profanity will be
prohibited on any part of the property.
9. The responsibility of the use of the facility will include but will not
be limited to: Facility
cleanliness; Cleanliness of any rest room(s) used; All trash
placed in dumpster. All
lights are to be turned off and doors locked.
